Using modern technology to keep patients connected to a team of RN Case managers and Remote Care Coordinators, we monitor and manage the patient’s health status from their home, enhancing the ability to identify trends for early intervention.

Why is Telehealth Important for Patients?

Telehealth equipment provides instant video access to a care coordinator who then connects with an RN case manager when appropriate. Telehealth monitoring is capable of checking weight, blood pressure, blood glucose, oxygen levels, and automatic medication reminder alerts ordered by a doctor or nurse.
