Wilshire Connected Care
Wilshire Connected Care is a multidisciplinary team of clinicians and care providers who support and treat patients primarily at residential facilities, limited home visits or through our remote patient monitoring. Our primary goal is to provide patients with compassionate, quality care regardless of mobility, transportation or access.
